29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Entire worksite, ground area, on or about 10-23-19:  Two employees, without head protection, were working below an employee working above them.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Main roof, on or about the week of 10-14-19 through 10-18-19:  Employees were working on a sloped roof, 10:12 pitch, and were not protected from falls, exposing the employees to a fall hazard of 20 feet from eave to ground.     b) West side of home, rear porch, on or about 10-23-19:  Three employees were working on a sloped roof, 3:12 pitch, and were not protected from falls, exposing the employees to a fall hazard of 14 feet from eave to ground.
