29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:    Seven Brothers Painting Inc. worksite located at 700 Cedar St., Fremont, Ohio 43420:  On or about October 19, 2019,   employees were removing materials and equipment from the condensation floor of the Fremont Water Tower without the benefit of an accident prevention program to ensure employees were protected from fall hazards of 20 feet to the ground below through proper utilization and use of fall protection and ladders.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or person       Seven Brothers Painting Inc. worksite located at 700 Cedar St., Fremont, Ohio 43420:  On or about October 18, 2019, the employer did not ensure employees were protected from falls while climbing outside of a fixed ladder cage to dislodge an Air Spyder that was lodged in the fixed ladder cage. The employee was exposed to a fall distance of approximately 20 feet to a concrete surface.

The 2019 300 log did not define the object or substance that directly injured the employee.
