29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On 10/24/2019, employees of Jays Construction LLC were working on a residential building located at 1720 Woodridge Drive, Florissant, MO.  Employees were using a Metabo HPT NV45AB2 coil roofing nail gun without the use of eye protection.

29 CFR 1926.501(b)(13):  "Residential construction." Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On 10/24/2019, employees of Jays Construction LLC were working on a residential building located at 1720 Woodridge Drive, Florissant, MO.  Employees were installing shingles approximately 19 feet above a lower level.  Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards:  On 10/24/2019, employees of Jays Construction LLC were working on a residential building located at 1720 Woodridge Drive, Florissant, MO.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.
