29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Jobsite: On October 16, 2019 and at times prior thereto a guardrail system was not in place at two (2) locations in an area where employees staged work materials and equipment.  The 23 inch and 29 inch openings exposed employees to a ten foot fall hazard to the level below.    Note) Corrected during inspection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Attic area: On October 16, 2019 and a times prior thereto employees used a 10 foot A-framed ladder for access in/out of an 11.7 foot high window opening.    Note) Certification of corrective action is required for this item.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and stairways had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders:    a) Jobsite: On October 16, 2019 and a times prior thereto employees did not recognize or acknowledge the hazard of using a 10 foot ladder for access in/out of an 11.7 foot high window opening.  Actions were not taken to correct of minimize the hazard.        Note) Certification of corrective action is required for this item.
