29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) 457 W Lincoln Street, Easton, PA, Worksite - on or about October 25, 2019, employees were performing work from the fourth floor with unguarded edges, approximately 50 feet above grade, without utilizing any means of fall protection.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103)  a) 457 W Lincoln Street, Easton, PA, Worksite - on or about October 25, 2019, employees were performing demolition work while voluntarily using 3M half mask respirators without being provided with a copy of Appendix D prior to their initial use.
