29 CFR 1910.176(b): Storage of material created a hazard: Warehouse: Employer is exposing employees to hazardous storage conditions. The employees are permitted to work where damaged storage racks are in use. On 10/28/2019 employees were exposed to unsafe storage condition where storage racks were not fastened to workplace floor and damaged uprights were in use for material storage.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Warehouse: Employer is exposing employees to struck-by conditions. Operators of forklifts are not trained in the operation of forklifts they operate daily. On 10/28/2019 operators of forklifts were not provided required training prior to being permitted to operator forklifts.
