29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Market Place Vista Apartments, Roof: On or about 10/29/2019, and at times prior, an employee was exposed to fall hazards on a steep 6/12 pitched roof at eave heights of 42-feet, 6-inches, in that the employee was not trained to install and don a complete personal fall arrest system when exposed to fall hazards. Training by a competent person was not provided or documented as the fall protection systems used on site were not adequate.
