29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)   On October 29, 2019, for employees exposed to fall hazards of approximately 36 feet when removing shingles on a 6:12 pitch roof at 135 Ivy Avenue in Bismarck, North Dakota.

29 CFR 1926.1053(a)(27): Individual-rung/step ladders, did not extend at least 42 inches (1.1 m) above an access level or landing platform either by the continuation of the rung spacings as horizontal grab bars or by providing vertical grab bars that shall have the same lateral spacing as the vertical legs of the rungs:    (a)   On October 29, 2019, for employees using  ladders to access the roof of a three story building to remove shingles where the ladders did not extend one rung over the top at 135 Ivy Avenue in Bismarck, North Dakota.
