Employees were observed using pneumatic nail guns without the use of protective eyewear.

Employees were operating a ROLAIR compressor, (serial #14011568) in which the guard for a rotating shaft on the engine was removed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a) At a worksite site located at 6140 Rockhill Rd, Kansas City, MO: Employees were exposed to fall hazards. Two employees were engaged in roofing work on July 11, 20119 and were not protected from falling by the use of guardrail systems, safety net system, or personal fall arrest system. Employees were exposed to a fall of approximately 9 feet to the ground below.  b) At a worksite site located at 6140 Rockhill Rd, Kansas City, MO: Employees were exposed to fall hazards. One employee was engaged in roofing work on July 12, 2019 and was not protected from falling by the use of guardrail systems, safety net system, or personal fall arrest system. Employees were exposed to a fall of approximately 9 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A worksite site located at 6140 Rockhill Rd, Kansas City, MO: Employees were exposed to a fall hazards. Employees were using GORILLA brand extension ladders to access the roof of the residential worksite. The extensions ladders were not extended at least 3 feet above the upper landing surface. The ladders were placed only two feet above the upper landing of the roof.
