On or about October 28, 2019, employees operating drills to cut aluminum were not provided eye protection and was being exposed to flying debris.

On or about October 28, 2019, two employees working from the JLG Lift were not wearing body belts and lanyards attached to the basket and were being exposed to fall hazards up to 20 feet.
