29 CFR 1926.407(b): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not approved as intrinsically safe or approved for the hazardous (classified) location or safe for the hazardous (classified) location:    (a)  At the worksite, on or prior to 10/30/19:  Employees were using a Milwaukee Fuel 18v 1/2" Cordless Hammer Drill and Milwaukee Fuel 18v cordless reciprocating saw (Sawzall) to cut holes in FRP tanks containing residual gasoline tank sludge and vapors; tools are not approved for use in a Class I Division II environment, exposing employees to fire and explosion hazards.

29 CFR 1926.651(g)(1)(iv): When controls were used that were intended to reduce the level of atmospheric contaminants to acceptable levels, testing was not conducted as often as necessary to ensure that the atmosphere remained safe:      (a)  At the worksite, on or prior to 10/30/19:  While preparing a gasoline UST for removal, including excavation of the tank and opening the tank for ventilation, the employer did not conduct testing to ensure that concentration of flammable vapors in the excavation did not exceed a safe level, exposing employees to fire and explosion hazards.
