29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     On 11 June 2019, at a worksite located at 204 Long Beach Rd, Oswego IL, 60543, employees who were removing and replacing a roof on a residential structure were exposed to fall hazards of approximately 23 feet. Some employees were wearing fall protection harnesses but were not tied off, while others were not wearing any form of fall protection. No other form of fall protection was being utilized during this work activity.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    On or about 11 June 2019, at a worksite located at 204 Long Beach Rd, Oswego IL. 60543, employees who were removing and replacing a roof on a residential structure were not trained adequately to recognize the hazards of falling and the steps to mitigate those hazards. No form of fall protection was being utilized during this work activity, exposing employees to falls of approximately 23 feet.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 11 June 2019, at a worksite located at 204 Long Beach Rd, Oswego IL. 60543, employees of Ocampo Construction Inc., who were engaged in residential roofing operations were using an extension ladder that was not extended to at least three feet above the eave to allow for safe access to the roof. This exposed employees to a fall of approximately 23 feet feet onto concrete below.     Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
