29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  General Production Area, Slitter Machine #310.  On or about November 1, 2019, employees who performed blade changes on the slitter machine were exposed to amputation and laceration hazards from unexpected startup and/or release of stored energy.  Employees were instructed to turn off the machine, but not to deenergize and lock out the machine's energy sources.  Employees were not required to control hazardous energy sources in the following sequence:       1) turn off and shut down the machines,      2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),  3) affix lockout or tagout devices to each energy isolating device,      4) relieve, disconnect, restrain and otherwise render safe all hazardous residual or stored energy,    5) verify that isolation and de-energization of the machines or equipment have been accomplished.       Alternative protections, such as machine guarding, were not utilized.   There is no abatement documentation or certification required for this item.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:     General Production Area, Slitter Machine #320.  On or about November 1, 2019, employees were exposed to injuries such as amputations and lacerations from exposure to the unguarded rotating slitter blades.  The moveable (hinged) barrier guard was not adequate to prevent entry into the danger zone during the operating cycle.   The guard interfered with the operator's access to the machine controls when it was in its intended position, and the machine was operated with the guard in a raised position.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
