29 CFR 1910.146(d)(3)(vi): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices to verify that conditions in a permit space were acceptable for entry throughout the duration of an authorized entry:  (a)  Simplot, Don Plant, Boiler #1:  On or about October 25, 2019, the controlling employer, J.R. Simplot Company, while conducting steam blowouts on the super heaters, inadvertently heated up the soda ash wash and it condensed out into the boiler and exposed sub-contracted Guy Nielson Company employees to respiratory illness.   Note:  Abatement certification IS required for this item
