A check on column (J) was not entered, on or about 11/6/19.  b) Oaks Senior Living, LLC, Cumming, GA - On 10/8/19, an employee fell in the kitchen and cracked a tooth, which required treatment. A check on column (J) was not entered, on or about 11/6/19.
