29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  A) Building B east side exterior wall , on or about November 7, 2019, and or times prior to, employees were using an electrical receptacle box that was improperly wired directly to the main wires of a breaker panel; the breaker panel did not have a dead front panel; the breaker panel box was not secured to the exterior wall; a receptacle outlet cover was not provided for the receptacle; and the receptacle box was not designed to be used outdoors and was not secured to a fixed surface, exposing employees to electrical shock hazards.

29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph.   A)  Building B east side exterior wall, on or about November 87, 2019, and or times prior to, exposed energized parts in a breaker panel were not protected against contact, exposing employees to electrical shock hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   A)  Building B east side wall, on or about November 7, 2019, and or times prior to, an employee was working from a pump jack scaffold that did not have a mid-rail on the back side of the scaffold platform or guardrails on the open ends of the platform, exposing the employee to a 15 foot fall hazard.
