In the shipping and receiving room, the employer did not ensure the exit door used by employees as an exit route swung outward in the direction of the exit discharge for quick exit of the building in case of an emergency exposing employees to fire and smoke hazards.

In the Lamination room on the south wall, the employer did not ensure the electrical panel boxes used to energize the lamination machines, lights and other electrical equipment were effectively closed during operations. 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).   In the Lamination room, the employees used an Pelone heater and floor fan connected to an extension cord missing the ground pin.  The employees were exposed to electrical shock hazards.
