29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a jobsite located at 129 South Stewart Road, Liberty, Missouri 64068, an employee was exposed to serious struck-by hazards. An employee was working with a hand shovel underneath the Kubota U55-4 excavator bucket while it was removing debris from the trench. The employee was not wearing a protective helmet to prevent a possible head injury from falling debris.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  At a jobsite located at 129 South Stewart Road, Liberty, Missouri 64068, employees were exposed to trench collapse and cave-in hazards. A ramp that the employees were using for egress was located 38 feet from where the employees were working.

At a jobsite located at 129 South Stewart Road, Liberty, Missouri 64068, employees were exposed to trench collapse and cave-in hazards. Employees were observed working in an unprotected trench without proper sloping, shoring, benching, or shielding to prevent collapse.
