At the jobsite located at 2050 N. Webb Rd. Wichita, KS:  Employees engaged in residential roofing activities were exposed to struck-by hazards in that the employer did not certify that the forklift operator had been trained and evaluated.

29 CFR 1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Employees exposed to fall hazards.  At the jobsite located at 2050 N. Webb Rd. Wichita, KS:  Employees engaged in commercial roofing activities were exposed to fall hazards of approximately 34 feet from the eave of the three story building to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing.
