29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions:       a) On or about November 6, 2019, employees were not provided with appropriate foot protection to guard against punctures and lacerations to the feet when required to work on and in roofing debris containing nails.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:             a) On or about November 6, 2019, employees were exposed to the hazard of falling approximately 16 feet where a portable extension ladder used to access the roof elevation of the building did not extend above the landing surface.   No abatement certification or documentation required for this violation.
