On or about November 5, 2019, An employee was exposed to fall hazards on a job site in Kewanee, Illinois, when working off of a Altec Telescopic Aerial Device, and adequate fall protection was not being used in accordance with 29 CFR part 1910, Subpart I.

On or about November 12, 2019, An employee working off a Altec Telescopic Aerial Device is required to wear fall protection, and the employer did not document a personal protective equipment hazard assessment. The written certification did not include the name of the person who certified that the evaluation had been performed, and the date that the hazard assessment was completed.
