29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At Hotel: On or about 10/30/2019 and at times prior, the employer exposed employees to struck-by hazards, in that head protection was not utilized by employees while  performing electrical work on the 3rd floor deck.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  At Hotel: On or about 10/30/2019 and at times prior, the employer exposed employees to fall hazards of approximately 20 feet, in that fall protection was not utilized by employees while  performing electrical work at the edge of the 3rd floor deck.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:   At Hotel: On or about 10/30/2019 and at times prior, the employer exposed employees to impalement hazards, in that rebar on the 2nd and 3rd level were not equipped with protective caps.
