29 CFR 1926.20(b)(2): Frequent and regular inspections of the job sites, materials, and equipment were not made by a competent person designated by the employer:   a) Job site, corner of Eleanor and Eccles Streets, Pittsburgh, PA: On or about November 13, 2019, during roofing work, the employer did not ensure that an appropriate system was in place to recognize and correct safety concerns at the job site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection where a fall hazard of up to approximately 20 feet existed, not ensuring that personal protective equipment was used such as hardhats or safety glasses and not making sure that sound ladder safety practices were followed.
