(a)    Special Education Room: The room was not kept clean and sanitary as there were dead bugs, cobwebs, and dirt and debris along the floors, window sills and walls; exposing employees to health hazards.  (b)    Special Education Storage Room: The room was not kept clean and sanitary as there was water damage and mold observed on the shelves; exposing employees to health hazards.  (c)     Hallways: The areas were not kept clean and sanitary as there was water damage to ceiling tiles, mildew and staining from dirt located under the water fountains, and dead bugs, cobwebs, dirt and debris along the floors, window sills and walls; exposing employees to health hazards.  (d)    Staff Lounge: The room was not kept clean and sanitary as there was water damage to ceiling tiles, visible mold growth near the air vent and dead bugs, spiders and cobwebs, and dirt and debris along the floors, window sills and walls; exposing employees to health hazards.  (e)    Kitchen: The room was not kept clean and sanitary as there were dead bugs on the floor, and water damage and mildew/mold on the ceiling; exposing employees to health hazards.  (f)      Kitchen Chemical Storage Room: The room was not kept clean and sanitary and had trails of chemicals that had leaked onto the floor causing corrosion; exposing employees to health hazards.  (g)    Psychology Room: The room was not kept clean and sanitary and had termite mud trails along the walls, and dead bugs, cobwebs, dirt, and debris along the floors and window sills; exposing employees to health hazards. 29 CFR 1910.141(a)(5): Enclosed workplaces were not so constructed, equipped, and maintained as to prevent the entrance or harborage of rodents, insects, or other vermin:  (a) Southwest Exit Doors: Gaps underneath the exteriors doors near the records room were large enough that snakes, rodents, and scorpions were able to enter the building, exposing employees to injury from bites, stings and diseases. (b) Southwest Exit Doors: Gaps underneath the exteriors doors near the band room were large enough that snakes, rodents, and scorpions were able to enter the building, exposing employees to injury from bites, stings and diseases. (c) Northwest Exit Doors: Gaps underneath the exteriors doors at main entry were large enough that snakes, rodents, and scorpions were able to enter the building, exposing employees to injury from bites, stings and diseases. (d) Special Education Classroom:  Gaps underneath the two exteriors doors were large enough that snakes, rodents, and scorpions were able to enter the building, exposing employees to injury from bites, stings and diseases. (e) Special Education Storage Room: Holes in the ceiling near the electrical panel were large enough that rodents, scorpions, and insects were able to enter the building, exposing employees to injury from bites, stings and diseases. (f) Staff Lounge: Holes in the ceiling tiles were large enough that rodents, scorpions, and insects were able to enter the building, exposing employees to injury from bites, stings and diseases. (g) Southwest Hallway: Missing ceiling tiles near the concession booth created holes that were large enough that rodents, scorpions, and insects were able to enter the building, exposing employees to injury from bites, stings and diseases. (h) Psychology Room: Holes in the ceiling, where ceiling tiles did not align with the wall, created gaps that were large enough that rodents, scorpions, and insects were able to enter the building, exposing employees to injury from bites, stings and diseases. (i) Psychology Room Storage Closet: Holes in the ceiling were large enough that rodents, scorpions, and insects were able to enter the building, exposing employees to injury from bites, stings and diseases. (j)Kitchen: Holes in the ceiling near the walk-in coolers were large enough that rodents, scorpions, and insects were able to enter the building, exposing employees to injury from bites, stings and diseases.
