This violation was observed on November 15, 2019, when employees working from welded frame scaffold platforms were not protected from falling approximately 13 feet to the concrete below. A guardrail system or other form of fall protection was not provided.

29 CFR 1926.451(e)(1): The employer did not ensure that when scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was used. Cross braces shall not be used as a means of access. This violation was observed on November 15, 2019, where at least two (2) employees working on welded frame scaffolds without safe access to all levels. Employees were exposed to falls of approximately 13 feet to the concrete below.
