29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a) On or about November 15, 2019, at the building 2 of the construction site, employees climbed the frame of a tubular scaffold approximately 13-feet above the ground that was not provided with an access ladder while performing masonry work.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:  a) On or about November 15, 2019, at the northeast side of the construction site, employees were installing masonry bricks from a scaffold approximately 13-feet above the ground without installing toeboards along the edges of the platform to prevent objects from falling on workers on the lower level while performing masonry work.
