29 CFR 1910.219(c)(4)(ii): The unused keyway(s) of projecting shaft end(s) were not filled up or covered:  a)  Bakery, Bun Packaging Line Conveyor Belt Transition Point:  On or about November 13, 2019 and at times prior employees were exposed to caught in hazards while working around an unprotected keyed shaft located on the West side of the bun line (UBE) machine conveyor that protruded approximately 5/16 of an inch.  b)  Bakery, Bun Packaging Line Conveyor Belt Transition Point:  On or about November 13, 2019 and at times prior employees were exposed to caught in hazards while working around an unprotected keyed shaft located on the East side of the bun line (UBE) machine conveyor that protruded approximately 5/16 of an inch.
