29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about November 18, 2019, employees sheeting a steep commercial roof were not protected from falls, exposing the employees to a fall of 13.7 feet.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  On or about November 18, 2019, prior to operating a rough terrain powered industrial truck, the operator was not evaluated every three years as required by the standard, exposing the employee to the hazards associated with operating a rough terrain powered truck.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
