29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Jose Perez Roofing, LLC located at 6292 Bellowvalley Drive, Dublin, OH 43017:  On and preceding November 18, 2019, employees were removing shingles preparing for a re-roof on the roof of a two story residential home at heights up to 27 feet without fall protection or PPE. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jose Perez Roofing, LLC located at 6292 Bellowvalley Drive, Dublin, OH 43017:  On and preceding November 18, 2019, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were removing/installing shingles on a roof that varied from 10-27 feet with the employees working at different levels. The employees were not utilizing head or fall protection which exposed them to falls up to 27 feet and struck by hazards to the head.

Roofers were removing/installing shingles on a roof that varied from 10-27 feet with the employees working at different levels. The employees were not utilizing proper head protection which exposed the them to struck by hazards to the head.
