29 CFR 1926.453(b)(2)(v): A body belt shall was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) Job site, Sandusky and East Commons Street, Pittsburgh, PA: On or about November 15, 2019, during antenna installation work, the employer did not ensure that an employee, working in the basket of an aerial lift at a height of up to approximately 30 feet, utilized appropriate fall protection.
