29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    (a) At the back of the building:  On or about 11/19/19, employees worked from a carpenters' bracket scaffold that was not fully planked, exposing employees to a fall hazard of approximately 13' 8".    (b) At the back of the building:  On or about 11/15/2019, employees worked from a tubular welded frame scaffold that was not fully planked, exposing employees to a fall hazard of approximately 12'.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling:  (a) At the back of the residence:  On or about 11/15/2019, employees were working from a tubular welded frame scaffold that was approximately 2 1/2 feet from the face of the wall of the building, without installing guardrails along the front and back edge of the scaffold, or having the employees use personal fall protection systems.

(b) At the back of the building:  On or about 11/19/2019, employees worked from a carpenters' bracket scaffold that did not have proper guardrails and was missing a guardrail system at the ends of the scaffold, exposing employees to a fall hazard of approximately 13' 8".
