29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation. On or about November 19, 2019, and at times prior thereto, subcontractors were utilizing supported scaffolding which had legs not bearing on mud sills or other adequate firm foundation, exposing subcontractors to the hazard of falling from height.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. On or about November 19, 2019, subcontractors were accessing scaffolding platforms 9.5 feet high by climbing the scaffolding support poles and lower level platforms due to the lack of an appropriate point of access, such as an attachable ladder, exposing subcontractors to the hazard of falling from height.
