29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about November 15, 2019, the employer did not develop specific lockout/tagout procedures for the DoAll vertical blade saw, serial number 264-79295. An employee was attempting to repair a water hose on the machine while the saw was running where the substantial amputation of the left hand ring finger and index finger.    No abatement documentation is required for this item.
