On 11/8/19, at the 2311 Sparta Way, Braselton, Georgia job site, employees performed mason worked from scaffolds and were exposed to fall heights greater than 15 ft. to lower levels as guardrails systems were not installed along all open sides and ends of the scaffolds.

On 11/8/19, at the 2311 Sparta Way, Braselton, Georgia job site, employees performed mason work from scaffolds that were not equipped with cross braces at all inner and outer supports.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On 11/8/19, at the 2311 Sparta Way, Braselton, Georgia job site, employees performing mason work were climbing scaffold frames to access working levels as there was no ladder provided to access those levels.

On 11/8/19, at the 2311 Sparta Way, Braselton, Georgia job site, employees performing mason work, were permitted to work from scaffolds that had not been inspected by a competent person and had various deficiencies including lack of fall protection, fully decked working levels, access ladders and cross bracing.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On 11/8/19, at the 2311 Sparta Way, Braselton, Georgia job site, the employer did not create a training program so as to train employees to identify and eliminate hazards when working from and on scaffolds.
