29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Jaime Calvillo located at 4470 Habersack Ave. Obetz, OH 43207: On and preceding November 21, 2019,  employees were installing shingles on a re-roof project of on a single story residential home at heights up to 18 feet without fall protection or PPE. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jaime Calvillo located at 4470 Habersack Ave. Obetz, OH 43207: On and preceding November 21, 2019,  the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were installing shingles with pneumatic nail guns on a roof that varied from 10-18 feet. The employees were not utilizing eye or fall protection which exposed them to falls up to 18 feet and struck by hazards to the eyes.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Jaime Calvillo located at 4470 Habersack Ave. Obetz, OH 43207: On and preceding November 21, 2019, the employer did not ensure that employees wore eye protection while utilizing pneumatic nail guns. Workers were installing roofing shingles using pneumatic nail guns powered at 90-120 psi without the use of eye protection.  The following condition exposed employees to struck-by hazards to the eyes.

Roofers were installing shingles on a roof that varied from 10-18 feet. The employees were not utilizing proper fall protection which exposed them to falls up to 18 feet.
