29 CFR 1910.262(c)(5): Inspection and maintenance. All guards and other safety devices, including starting and stopping devices, shall be properly maintained.  (a) Plant Lines 1, 2, & 3: On or about 11/04/2019 and at times prior the employer failed to ensure panels, which acts as a guard on the Davis & Furber Pillow stuffer conveyor table rollers was attached prior to allowing employees to operate the pillow stuffer machine.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    (a) Throughout the facility On or about 11/04/2019 the company did not report a three day in-patient hospitalization of an injured employee within twenty-four hours.  OSHA was only notified due to a complaint filed on 11/19/2019, 15 days after the hospitalization occurred.
