On or about December 19, 2019, in the shop, employees were exposed to hand injury hazards while they poured and troweled concrete in trench molds without being required to wear proper gloves.    b.  On or about December 19, 2019, in the shop, employees were exposed to hand injury hazards while they patched concrete trenches with a cement-based concrete and repair mortar with without being required to wear proper gloves.

On or about December 19, 2019, employees in the southeast corner of the shop were exposed to chemical hazards while pouring and troweling concrete without a suitable eyewash station.    b.  On or about December 19, 2019, employees on the north side of the shop were exposed to chemical hazards while pouring and troweling concrete and access to a suitable eyewash station was blocked by equipment.

29 CFR 1910.1053(c): The employer did not ensure that no employee was exposed to an airborne concentration of respirable crystalline silica in excess of 50 ug/m3, calculated as an 8-hour TWA:    In the following instances, three employees were exposed to silica in excess of the permissible exposure level (PEL) for the 8-hour time weighted average concentration:    a.  An employee who was conducting concrete grinding operations was exposed to silica at a time weighted average of 191.6 ug/m3.  This level exceed the permissible exposure level for silica of 50ug/m3 by 3.83 times.  This exposure was derived from samples collected over a 464 minute sampling period on December 19, 2019.    b.  An employee who was conducting concrete grinding operations was exposed to silica at a time weighted average of 67.1 ug/m3.  This level exceed the permissible exposure level for silica of 50ug/m3 by 1.34 times.  This exposure was derived from samples collected over a 461 minute sampling period on December 19, 2019.    c.  An employee who was conducting concrete grinding operations was exposed to silica at a time weighted average of 143.6 ug/m3.  This level exceed the permissible exposure level for silica of 50ug/m3 by 2.87 times.  This exposure was derived from samples collected over a 459 minute sampling period on December 19, 2019.

29 CFR 1910.1053(h)(2)(i): The employer allowed compressed air to be used to clean clothing or surfaces where such activity could contribute to employee exposure to respirable crystalline silica without being used in conjunction with a ventillation system to capture blown dust.    On or about November, 25, 2019, employees were exposed to silica hazards when using compressed air to clean and remove debris from the inside of concrete trenches after they ground the edges of the trench.
