29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:    On or about November 22, 2019, an employee was exposed to fall hazards approximately five stories to the ground below when working from a Genie S-85 Articulating Boom Lift and the employer did not ensure a suitable fall protection system was used.

29 CFR 1910.140(c)(7): D-rings, snaphooks, and carabiners were not capable of sustaining a minimum tensile load of 5,000 pounds:  On or about November 22, 2019, an employee was exposed to fall hazards approximately five stories to the ground below when working from a Genie S-85 Articulating Boom Lift and the employer did not ensure the spring snap and quick link used to attach the harness to the anchor point was rated appropriately.
