29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    On or about 11/19/19, during building exterior cleaning and painting activities, located at 1001 Market Avenue North, in Canton, Ohio, the employer:     a) Failed to ensure that employees were protected from falling through the skylights. A fall height from the skylights to the surface below was about thirteen feet (13'), and    b) Failed to ensure that employees were protected from falling through the roof vents. A fall height from the roof vents to the surface below was about twenty-nine feet (29').
