29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451 (b)( 1 )(i)-(ii)    a)     POLLOCK PLASTERING INC. On or about November 26, 2019, at a job site location near 3301 Secor Rd, Toledo, OH: The employer did not ensure the scaffold platform being used by employees was fully planked.  Employees were performing plastering and exterior coating operations up to approximately 13 feet 6 inches above ground level working from a tubular welded frame scaffold with only one plank on each level to access the south and east side of the building.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.     a)     POLLOCK PLASTERING INC. On or about November 26, 2019, at a job site location near 3301 Secor Rd, Toledo, OH: The employer did not ensure that employees performing plastering and exterior coating operations up to approximately 13 feet 6 inches above ground level working from a tubular welded frame scaffold were protected from fall hazards. Employees were performing work on the east and south side of the building at heights ranging up to 13 feet 6 inches above ground level without fall protection or guardrails.

29 CFR 1926.451(h)(1): The employer did not ensure that in addition to wearing hardhats each employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.     a)   POLLOCK PLASTERING INC. On or about November 26, 2019, at a job site location near 3301 Secor Rd, Toledo, OH: The employer did not ensure that a system to prevent objects and materials from falling was installed, exposing employees to struck by hazards. Employees were wearing hard hats but were observed taking them off while on scaffolding at times and while performing plastering and exterior coating operations.
