OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards:  a) On or about November 26, 2019 at 2201 Glenwood Ave SE Atlanta, GA 30316.; the employer did not ensure that the outriggers of a 2018 Everdigm ECP41ZX 41M Pump (SN 1014) mounted on a 2018 Mack MRU613 (VIN #1M2AV04C9JM018450) were not over extended while poring concrete of a concrete slab thus exposing the operator and employees to crushing injuries associated with tip over hazards.
