29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  (a) Jobsite, Ellsworth Avenue, Pittsburgh, PA:  On or about 12/04/2019 no fall protection systems were in place to prevent an employee from falling into the excavation.  An employee was standing on the spreaders of the trench shield exposing them to a fall hazard of approximately 8 feet.

29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields:  (a) Jobsite, Ellsworth Avenue, Pittsburgh, PA:  On or about December 4, 2019 employees exited the shield and walked 1-2 feet to access the ladder when entering or exiting the work area exposing them to potential cave-in hazards.  The excavation had vertical side walls and was approximately 9 feet in depth.
