29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   Site: 1119 41st Street Brooklyn, NY  On or about 12/5/19  a) Employees accessed 6 tier high tubular welded frame scaffold which had no safe means of access. The employees climbed scaffold frames and cross braces to access their working levels.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee.  Site: 1119 41st Street Brooklyn, NY  On or about 12/5/19  a) Employees worked on a tubular welded frame scaffold from approximately 25 feet. They used the scaffold components as connection points.
