29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  (a) Upper Platform - On or about December 03, 2019 the employer did not install metal floor grating over two open holes along the walkway exposing employees to a fall hazard of approximately 80 feet.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members shall be installed between the top edge of the guardrail system and the walking/working surface when there is no wall or parapet wall at least 21 inches (53 cm) high.  (a) Upper Platform - On or about December 03, 2019 the employer did not install a midrail before two open holes along the walkway.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:  (a) Upper Platform Guardrail System - On or about December 03, 2019 the employer did not install a top rail designed to withstand a 200 pound load applied in the downward and outward direction exposing employees to a fall hazard.
