29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  On or about November 26, 2019 and at times prior, at 120 NW 27th AVE, Miami, Florida, 33315, employees were installing metal decking with an unprotected side or edge more than 15 feet (4.6 m) above a lower level and were not protected by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems; therefore exposing the employees to a fall hazard. 29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:  On or about November 26, 2019 and at times prior, at 120 NW 27th AVE, Miami, Florida, 33315, employees were installing metal decking, during which the employer did not provide training by a qualified person to each employee in fall hazard recognition, identification and prevention, and the use and operation of fall hazard prevention systems to be used, and the procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used, and the procedures to prevent falls through/into holes and use/limitations of the site-specific erection plan and hoisting, landing and placing of metal decking bundles; therefore exposing the employees to a fall hazard.   NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
