29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.    On or about November 20, 2019 employees were reported working on top of a commercial building installing ceiling corrugated sheet panels without any type of fall protection system. This condition exposed employees fall hazards from heights of 6 feet or more.

29 CFR 1926.602(c)(1)(vii): Unauthorized persons were allowed to ride on a powered industrial truck:  On or about November 20, 2019 employees were reported working on a commercial building installing wall and ceiling corrugated sheet panels where employees were riding on a wood pallet lifted by a skid steer power industrial equipment.  This condition exposed employees to struck-by and fall hazards from heights of 6 feet or more.
