29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  (a)  On or about December 6, 2019, workers operating a Makita skill saw on the south side of building C at the Patriot Park apartment construction site, were exposed to shock and burn hazards from the extension cord powering the saw in that the insulating sheath was not securely attached to the plug thus allowing strain being placed directly on the terminal screws and joints.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
