29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.    a. On or about December 4, 2019, the employer did not ensure an employee utilized the developed and documented procedures for the control of hazards energy while repairing the Line #5, Loveshaw, Little David Model, box taping machine.       b. On or about December 10, 2019, the employer did not ensure an employee utilized the developed and documented procedures for the control of hazardous energy while setting up the Line #5, Loveshaw, Little David Model, box taping machine.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:   a. On or about December 4, 2019, the employer did not ensure an employee an employee making repairs to the Line #5, Loveshaw, Little David Model, box taping machine performed and followed the established energy control procedures by; isolating the machine from the electrical and pneumatic energy source(s) [per 1910.147(d)(3)], affixing a lockout device [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to repairing the machine.       b. On or about December 10, 2019, the employer did not ensure an employee setting up the setting up the Line #5, Loveshaw, Little David Model, box taping machine performed and followed the established energy control procedures by; affixing lockout devices to the electrical and pneumatic energy isolating devices [per 1910.147(d)(4)(i)], prior to setting up the machine.
