OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  (a)  SWABB CO., dba SWABBCO, at Latitude 40.131299, Longitude -105.042743, Regnier Farms 19H-B268, Longmont, CO: On or about 12/9/19, SWABBCO did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to a fire hazard and a struck-by hazard.  Employees worked on a oil/gas well site performing well completion activities.  Employees assisted the drilling company in tripping out of the hole. Tripping out of the hole was conducted using a hybrid drill rig, using its top drive to pull the pipe out of the well. A snubbing unit was utilized to assist in disconnecting the joints as pipe was removed from the well bore.  Pipe light condition occurred on 12/9/2019. The top drive, instead of the snubbing unit, was used to trip pipe out of the hole while in pipe light condition. Well control was lost resulting in production pipe and the bottom hole assembly being blown from the well. Hydrocarbons were released and ignited, resulting in a large flash fire. Three employees experienced thermal burns and fractures as a result of the flash fire. These conditions exposed employees to struck by hazards and fire hazards.
