OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) SWABB CO., dba SWABBCO, at Latitude 40.131299, Longitude -105.042743, Regnier Farms 19H-B268, Longmont, CO: On or about 12/9/19, SWABBCO did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to a fire hazard and a struck-by hazard. Employees worked on a oil/gas well site performing well completion activities. Employees assisted the drilling company in tripping out of the hole. Tripping out of the hole was conducted using a hybrid drill rig, using its top drive to pull the pipe out of the well. A snubbing unit was utilized to assist in disconnecting the joints as pipe was removed from the well bore. Pipe light condition occurred on 12/9/2019. The top drive, instead of the snubbing unit, was used to trip pipe out of the hole while in pipe light condition. Well control was lost resulting in production pipe and the bottom hole assembly being blown from the well. Hydrocarbons were released and ignited, resulting in a large flash fire. Three employees experienced thermal burns and fractures as a result of the flash fire. These conditions exposed employees to struck by hazards and fire hazards.

29 CFR 1910.1200(e)(2)(i): At a multi-employer workplace, the employer's hazard communication program as implemented did not include methods the employer used to provide other employers with on-site access to a safety data sheet for each hazardous chemical the other employers' employees may be exposed to while working: a) SWABB CO., dba SWABBCO, at Latitude 40.131299, Longitude -105.042743, Regnier Farms 19H-B268, Longmont, CO: On and preceding 12/9/19, SWABBCO's hazard communication program as implemented did not include methods the employer used to provide other employers with on-site access to a safety data sheet for each hazardous chemical the other employers' employees may have been exposed to while working. SWABBCO worked on a oil/gas well site performing well completion activities with other employers. SWABBCO did not ensure that its written hazard communication program as implemented included the methods used to provide on-site access to SDSs for hazardous chemicals brought to the well site by SWABBCO that employees of other employers may be exposed to while working at the multi-employer worksite. Hazardous chemicals brought to the well site by SWABBCO included, but were not limited to the following: 1. Chevron Clarity Hydraulic Oil; and 2. Diesel fuel. b) SWABB CO., dba SWABBCO, at Latitude 40.131299, Longitude -105.042743, Regnier Farms 19H-B268, Longmont, CO: On and preceding 12/9/19, SWABBCO did not ensure that Safety Data Sheets (SDS) were readily accessible during each work shift to employees when they were in their work area(s).
