29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources:  a).  Main Shop:  On or about December 11, 2019, and at times prior thereto, employees were exposed to a caught-in hazard while operating a HAAS GR510 router with bypassed light curtains.    b).  O3 Building:  On or about December 11, 2019, and at times prior thereto, employees were exposed to a caught-in hazard while operating a HAAS GR712 router with bypassed light curtains.
